PERSONAL APPEARANCE
Notarization of documents that are not subscribed or acknowledged before the notary public is illegal. We will not notarize Deeds, Conveyances, Agreements, or Affidavits involving people who are already dead or located outside the country.
VALID IDENTIFICATION CARD
A person who wants to have his or her document/s notarized must show his valid government-issued ID so that we can verify his identity. We will ask you temporarily remove any mask/accessory for identification. The ID must contain the full name, picture, address, and signature of the party/ies.
KNOW OR EXPLAIN WHAT YOU NEED
Make sure you know what you need. Talk to the designated staff about what you need. Tell them what you need and where you will use it. The staff will ask for details and other information to complete the document.
RECHECK CORRECT ENTRIES
We will not notarize blank or incomplete documents. Fill in your forms properly. If the document is prepared by the Office, you will be given the opportunity to check for clerical errors. Check carefully to avoid inconvenience.
SIGNING/ THUMBMARK
The affiant must sign or mark the notarial document in front of the notary public. He must only do this if he understands the contents of the document and that his action is voluntary. Otherwise, the document shall not be notarized.
PICTURE TAKING
The parties in a Deed or Conveyance must personally appear. After the signing of the documents, they will have to pose for a picture to prove personal appearance for documentation purposes.
OFFICE COPIES
We collect from the client at least one (1) copy for affidavits and three (3) copies for acknowledgments. They will be kept for record purposes and court submission.
PAYMENT OF FEES LEGAL
We do not overprice our charges. We comply with the legal fees for notarization provided by the Integrated Bar of the Philippines - Tarlac Chapter and the Supreme Court which were last updated in 1994.
DON'T FORGET YOUR FILES
Before leaving the Office, check the notarial act. It must be signed by the lawyer and stamped with dry seal. Check all your documents and personal belongings especially identification cards and original papers. We are not responsible for any missing documents or ID.

Overseas Workers Welfare Administration (OWWA) E-Card
Commission on Elections (COMELEC) Voter's ID or Voter's Certificate issued from COMELEC main office in Intramuros, Manila (Certificates issued in COMELEC's regional offices may require further verification. Applicants are requested to present NBI or police clearance along with the Voter's Certificate).
Philippine National Police (PNP) Permit to Carry Firearms Outside Residence
Senior Citizen ID
Airman License (issued August 2016 onwards)
Philippine Postal ID (issued November 2016 onwards)
Seafarer's Record Book (SRB)(*must be issued Feb 2020 onwards) issued by the Maritime Industry Authority (MARINA)
Valid or Latest Passport (For Renewal of Passport)
For minor applicants, School ID OR Certificate of Enrolment with photo of minor and dry seal of school may be presented
